FOOD COMMITTEE
The Executive Committee recommended Bruce Handley, Mike
Palumbo, Alma Harris Joe Horvat, Bill McVaugh, and Yu-Jane Yang as
possible faculty representatives on the Food Committee.

This policy goes into effect on Monday, October 2, 2000. The
Executive Committee discussed at length the policy in general and the
$25.000 deposit required for keys.
HONORARY
DEGREE
The deadline for Honorary Degree recipients is tomorrow,
September 29, 200 at noon. In the past there has been a desire that
one on the recipients be a faculty member or former faculty member.
Gene Sessions indicated that the Honorary Degree Committee was meeting
this afternoon and would submit suggestions.
FACULTY
EVALUATIONS
Sample faculty evaluation forms from the University of
Utah, Utah State University, Southern Utah University, and Weber State
University College of Social and Behavioral Sciences were reviewed and
discussed.
DATA
COLLECTION
The question was raised regarding who would bare the
cost of running the data and compiling the information.
The Provost indicated that there should not be any additional
expense for departments as they are currently printing evaluation
forms and, adding two additional questions to their form, will not add
additional expense to the department. Compilation costs should be
minimal.
MOTION
Marie Kotter: Moved to have departments use a
generic evaluation form which contains two standard
questions or use a department evaluation form and add the
two standardized questions regarding the course and the
instructor.

Outcome: The motion passed unanimously.
STANDARD
QUESTIONS
The committee discussed using the two questions developed
by the Teaching and Learning Forum:

Gary Dohrer: Moved to approve the two standard
questions stated above.

Outcome: The motion passed unanimously.
RELEASE
OF DATA
Release of information contained in faculty evaluations is
required by the Government Records Access and Management Act (GRAMA).
DATA
COMPILATION
Dave Eisler will contact the Institutional Research
Office with regard to compiling the data.
The Executive Committee discussed a process for making available faculty
evaluation information.
PROCESS PROCESS FOR POSTING FACULTY EVALUATION DATA
Executive Committee members will discuss the process with faculty within their college.
Executive Committee members will discuss the process with Department Chairs.
The Provost will present information at Dean=s Council.
As an agenda item, Posting Faculty Evaluation Data, will be discussed at the October 19, 2000 Faculty Senate meeting.
Current policy (PPM 8-11) regarding faculty evaluations will be followed.
A generic faculty evaluation form including two standardized questions will be distributed this Fall to all departments. Departments can use the generic evaluation form or add the two standardized questions to their department faculty evaluation form.
The evaluation data from the two standardized questions will be released by class, listing instructors.
A full distribution of student responses to the two questions will be released.
The information from the two standardized questions will be included in the tenure and promotion process.
Data from Fall and Spring Semester evaluations will be compiled through the Institutional Research Office and made available at the Reserve Desk in the Library.

HISTORY DEPARTMENT
Lee Sather, Chair of the History Department, has written
a memo regarding the department=s
position on the use of C.E. and B.C.E. in the title of History 4210 A
copy of Dr. Sather=s
memo will be forwarded to Executive Committee members.
NEXT MEETING
The Executive Committee will meet on October 5, 2000 to
discuss Dr. Sather=s
memo and the position of the History Department, and to finalize the
process regarding the release of faculty evaluation data.
ADJOURN
The meeting adjourned at 3:40 p.m.
