Instructions for Applying for the Emergency Medical Services Certification (Utah Paramedic Certification)

 

Please Read These Instructions Carefully!!

 

1. To obtain an application, click on the link below and print off a copy to fill out.   There are two forms you need to fill out, so make sure that you download both of them.

 

2. Completely fill out the front page (including signature and date).

 

3. Do not sign the  page until you have the form notarized.  Most credit unions and banks will have an authorized notary.

 

 

 

4.     The fee to be included with the form is as follows:

 

1.     Check or Money Order made out to BEMS   

 

2.     $137.00 if you have lived in the State of Utah for the last five years; $166.00 if you have lived outside the State of Utah in the last five years.  (Military or mission time is excluded).

 

3.     If you have lived outside the State of Utah in the last five years, you will need fingerprint cards.  This can be done at most police stations.  That must be done at your own expense.

 

4.     Be sure to include two passport pictures with your application.

 

5.     The form, check or money order, passport pictures and fingerprint cards need to be sent to Mark Francom no later than July 8, 2004.
