Consent form
Please read the following before proceeding. The following describes your rights as a "human subject".
PURPOSE:
I am being asked to participate in a study that will examine my conceptions of science as a result of participating in undergraduate research. A researcher, Adam Johnston (faculty member of the physics department), will analyze student responses to inform how undergraduate researchers' ideas about science change as a result of a summer research experience.
PROCEDURES:
Undergraduate researchers will also be asked to type response to a brief a written questionnaire regarding their experiences in the research setting. A follow-up interview may be scheduled as well. I understand that my participation in any interview or questionnaire is voluntary and is not a requirement for any course or for my research experience.
DURATION:
Questionnaires should take less than 30 minutes to complete. Any interviews following the questionnaire will require less than 30 minutes, and will only be scheduled if necessary and as works with a student's schedule.
CONFIDENTIALITY:
My identity will remain confidential and will not be revealed in the rough transcripts of the interviews or in any published results of this study. As a participant in this study, I will be given a pseudonym and all information regarding my participation will be designated by my pseudonym. The key of names and pseudonyms will be kept in a secure and confidential place by the researcher and will not be made available to any other individual.
RISKS:
The researcher anticipates that my participation in this study will not result in any personal risks on my part. The faculty who mentor undergraduate researchers have no knowledge of any personal information revealed by this study. Steps have been taken to ensure participant confidentiality and research has been designed to minimize the chance of any personal risks.
BENEFITS:
I will receive a gift certificate in return for my participation in this study. There will not be any other direct benefit to me beyond the implicit benefits of reflection on science and the nature of scientific research.
WITHDRAWAL:
I recognize that my participation in this study is voluntary. Thus, I may refuse to participate in any questionnaire or interview at any time. Even if I agree to participate in the research efforts, it is my right to refuse to answer any questionnaire or interview question asked of me at any time, and this refusal will not impact my undergraduate research nor my reputation as a student at Weber State University.
CONCERNS:
