INFORMED CONSENT

Thank you for agreeing to participate in this research study.  Your research participation involves completing a questionnaire.  The questionnaire poses a series of 11 general questions about yourself, 20 questions about your learning styles, and then 12 questions about your reading and judgments of the  significance of  English.  The questionnaire should take about 20 minutes to complete.

We also ask your permission to retrieve university data about you, including the final grade in the psychology class in which you are now enrolled.  The institutional data will be delivered anonymously, by requesting the data according to the last four digits of your student "88" number.

You are able withdraw from the research for any reason at all and without any penalty.  All responses will be held in the strictest confidence. While your answers will not count towards your grade in this course, students completing the research will receive Research Credit at the discretion of your professor.  If you would like to receive the Research Credit but not complete the research, please write an account of your experience learning psychology during the period of time this research (about 30 minutes) is completed by other students and send it to Eric Amsel (eamsel@weber.edu).

By signing below (which you must do to receive extra credit) you are stating that you have read and understand your rights. If you have any questions about this form or the research, please contact Eric Amsel at eamsel@weber.edu
